Telephone headset. If you make a lot of calls, I recommend a headset, not just for your cell phone, but for your office phone.
I love the freedom of movement, ability to type to take notes, and a can answer from up to 300ft away.
Dual monitors. At risk of branding myself a "Nerd", I love having dual monitors. Basically, you just hook two monitors up to your computer, then drag each program window to the monitor of your choice.
I keep email and my calendar open all day on my right monitor. That frees my main monitor focused on my current work. I use a 24" iMac and connected a 24" LCD monitor as the secondary monitor.
Aside from being a nerd, studies have shown dual monitors increases productivity by decreasing the amount of time spent switching between programs.
Google Apps. Google is FREE. I now run my email service, document sharing, calendar, alerts, and more through Google. Total cost? About 2 hours of set-up once. Now? FREE.
